Prior to your first meeting with Jessica please fill out the New Client Form and the Food/Mind/Body Journal below. During your first appointment, these forms will be reviewed together.

Food/Mind/Body Journal

The intention behind the Food/Mind/Body Journal is to begin to develop a stronger connection between what you are eating and doing in your daily life and how it makes you feel. You will monitor your food, drinks, mood, digestion, energy, sleep, and physical activity. When documenting please be as honest as possible. This is a judgment-free zone.

Recent Lab Work

While you are not required to have labs it is helpful to review any recent labs that you have from the past year. If you would like Jessica to review your recent labs please email them along with your Food/Mind/Body Journal to
